We understand that each person has an individual story and that your care should reflect your needs. Not everybody can go out and get help when they need it - sometimes the help has to come to you. That's where Choice Home Care comes in. We offer the following home health care services:
Skilled Nursing: Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) provide caring and high quality medical care at home. Nurses are a key part of home care services because of their knowledge of disease processes, health assessment skills and ability to motivate clients to rebuild confidence and work toward recovery. The intricacy of an individual's medical condition and required course of treatment determine whether care should be provided by an RN or can be provided by an LPN.
Home Health Aides: Home Health Aides are paraprofessionals who have received specialized training for certification as a Home Health Aide, and who meet State of Minnesota and Medicare standards.
Home Health Aides assist clients with Activities of Daily Living and other services as necessary to maintain the personal comfort of the client. Tasks include assistance with:
Companion/Respite Services: Companions provide companionship and comfort to individuals who, for medical and/or safety reasons, may not be left at home alone. Some companions may assist clients with household tasks, but most are limited to providing sitter services.
Homemakers: Homemakers perform light household duties such as laundry, meal preparation, general housekeeping and shopping. Their services are directed at maintaining patient households rather than providing hands-on assistance with personal care.
Occupational Therapy (OT): Occupational Therapists help individuals improve, develop or recover their ability to perform tasks in daily living (ADLs) and work environments. OTs help people improve basic motor functions and reasoning abilities and make recommendations for adaptive equipment for home safety.
Physical Therapy (PT): Physical therapists work to restore the mobility, strength, balance and function of individuals who are limited or disabled by physical injuries or after surgery. PTs often alleviate pain and restore the movement and function of injured muscles. They also teach clients and caregivers special techniques for walking and transfer.
Speech Therapy (ST): Speech-language pathologists diagnose and treat speech and language disorders that result in communication impairments. They also work with people who have swallowing difficulties.
Medication Options Program
The Medication Options Program (MOP) is an integrated, community based program serving adults with a serious and persistent mental illness who have a history of non-compliance with their psychotropic medications or are unable to maintain independent living because of their disability. Choice Home Care has provided Medication Options program services since 2002.
Medication Options program nurses provide medication set-up, delivery and administration, symptom and side effect monitoring and physical and mental health assessment and follow-up. Physical health assessment is an important part of program services because many Medication Options clients have co-occurring disorders or disease processes, some of which can be chronic conditions. Individuals are referred to the Medication Options Program by Olmsted County Social Services Adult Mental Health social workers.
